Job Description:
The Registered Nurse (contractual) will provide skilled nursing care for our Adult
Day Services. Their responsibilities include:
i. Provide skilled nursing care for participants based on home visit, assessments and
problems identification. This includes consultant, emergency services and
documentation within clinical records.
ii. Develop written plan of care for each participant in consultation with their
physician. Provides implementations, evaluations and updates in accordance to
current regulations.
iii. Leads, evaluates, guides, informs, and supervises assigned staff in collaboration
with the center director.
iv. Coordinates with necessary agencies and/or physicians to certify participants
through the Professional Service Review Organization, Adult Evaluation and
Review Services and Veterans Administration.
v. Reviews Activity Program and calendar accordingly. Involves and educates staff
in care planning implementation and ongoing participation review and progress.
vi. Administers medication and treatment during center operation hours, as ordered
by the participant physicians.
vii. Develop and maintain cooperative relationships with community agencies and
organizations. Educates community agencies and groups regarding the SHORE
UP! Adult Day Services program and goals.
viii. Attend and complete training sessions and/or staff meetings as required by
SHORE UP! Inc.
ix. Adhere to agency policies and procedures and other responsibilities as needed.
B. Position Qualifications:
• Bachelor's degree from an accredited college or university, with two years’ experience
in nursing.
• One year prior related supervisory experience.
• Must pass a criminal background check.
• Must keep CPR certification and RN license current.
• Must submit current PPD or chest X-ray with a negative result for TB.
• Must provide liability certificate of insurance.
C. Position Characteristics:
Salary: $150 per assessment
Hours: Monday through Friday, 8:30 am to 4:30 pm
